Go to www.factsmgt.com
Click Family Login then select FACTS Family Portal
Type in our district code: sms-la
Type in your username and password. If you have forgotten your username or password, please click on the link provided.
After logging in, click on the Family Information button in the left menu.
Click on the Enrollment/Re-enrollment button.
Online Enrollment system will open with a link to the enrollment packet. The online process should take approximately 15 minutes to complete. Your information will be saved if you need to quit and come back later.
The Instructions and Enrollment Checklist page of the enrollment packet contains supplemental enrollment forms that also must be submitted. Further instructions on these forms are provided online.

St. Mary’s School is an open admission non-public Catholic school. Admission may be denied to any applicant if their needs cannot be properly served at St. Mary’s School. This may occur when class size does not allow for proper placement of a student, when there is a need for highly specialized counseling or treatment, or when a student’s discipline record indicates a disruptive pattern, etc. St. Mary’s Catholic School administration also reserves the right to deny enrollment of any student due to parental non- compliance of rules and misrepresentation of child or family information (i.e. age, health information, past academic records, church affiliation, etc.).
